Update Capsule Training & Physical Information

If you already have an active membership and need to add or update your Capsule Training Date / Document or Date of Last Physical Exam / Document Upload(s) click the button above or:

1. Login to your account.
2. Click on "My Account"
3. Click on the tab "Licenses/Applications"
4. Click on the "Add/Edit Capsule Training & Health Info" button
5. Add/Edit your information and click on "Submit"
